Change Management:The words change management mean different things to different people. This first session tries to examine different aspects of change management from a Health & Safety viewpoint. Our guest speaker today is Steve Jackson the Business Support Manager at Hydrocarbon Resources. Steve also chairs the Best Practice Club's Special Interest Group (SIG) on Change Management, so is well qualified to present some elements of the subject at our meeting today.
Incident Investigation Best Practice:This second session will examine best practice investigation techniques in line with legal and regulatory requirements & obligations. Having an excellent investigation process should dramatically reduce the likelihood of incident recurrence. Our second speaker Vince Fisher (formerly of AWE) has had a long career in the petrochemical and nuclear industries working on major projects in the UK and Europe. In recent years he has turned his attention to Operational Experience Feedback – Learning from Experience – and was chair of the UK Nuclear Industry’s Learning Group on Operational Experience. He is now an independent advisor and training provider.

The session identifies the Change Management process at Hydrocarbon Resources. The process covers both the hard and soft sides (ie as well as engineering change, with risk assessment, hazop, close out etc, it also looks at the impact on people from a point of view of competence, control, comfort, capability, etc.)

Afternoon Topic - Workplace Investigation of Accidents/Incidents

This session will work through the Workplace Investigation process and check the legal and
regulatory requirements and obligations. It will give an overview of how Human Factors play
their part in incidents and how to look for the Root Causes of incidents. Finally, it will explore how learning can be extracted from the investigations undertaken and ensure the actions decided upon can help to prevent the incidents re-occurring.
